SPRING 2008 -  Database / MailMerge Self-Check


Directions: Use the following checklist to review your work befre submitting it to your Teaching Assistant (TA)  for evaluation. Do not submit this assignment to Sakai.

Does the Data Entry Form Picture include all fields of the datafile?
Does the revised, completed format of the database file include:

• Ten fields, defined and appropriately placed?
• Artist, Concert Date, Location, URL as text fields?
• Payment as a Radio Button Field?
• Record, Price, Number of Tickets, Processing Fee as Numeric fields?
• Concert Date as a Date field?
• Price, Processing Fee, and Total Cost formatted as currency?
• Total Cost as calculation field?
• Default Values and Value Lists if required?
• Tab order settings to provide easy movement through the fields while entering data?
• A footer with your name, section number, and assignment due date?

Does the completed datafile contain twelve complete records?
Do you have all required printouts, including:

•the data entry .png form picture (step 3)?
•the complete database file, in List mode (step 9)?
•three subsets of the datafile, using search criteria specified in step 10?
•a modified, sorted datafile (with changes noted in steps 11 and 12), in Report mode (step 13)?
•the Mail Merge form letter, including merge markers (step 14e)?
•three personalized form letters, one for each of the first three records in the datafile (step 15)?

 Have you stapled all sheets together, including a Cover Sheet with your name, section
            number, TA name,assignment (Database/Mail Merge), and due date (step 16)?
